What to Bring to Your Appointment
- Your current valid insurance card
- Driver’s license or valid photo I.D.
- Any relevant medical records, labs, and imaging reports
- Registration forms
For after-hours care, please contact (833)4-BEV-CARE and the operator will be able to assist you and transfer you to our on-call physician if needed.
In case of an emergency, please call 911 or go to the nearest Emergency Room.
We understand that it can be difficult to find health care or find treatment where one feels comfortable. Our providers speak English, Spanish, Chinese, Vietnamese, Tagalog and Korean. We also have a translation service for most languages. Please let our staff know what is your preferred language and we will make sure we accommodate your needs.
To arrange transportation from and to any of our clinics, please contact us during business hours and 24 hours in advance at (833)4-BEV-CARE. We will schedule a pick up and drop off through our contracted transportation services.
