Insurance: BeverlyCare accepts most insurance plans. If you do not have insurance, please ask to speak with our eligibility specialist to see if you qualify for any state/county public programs. Our staff will be able to assist you.

Sliding Fee Schedule: For insured and uninsured patients, BeverlyCare offers a sliding fee discount schedule based on patient’s ability to pay. Please speak to our staff to assist you with the process.

If this is your first appointment, please bring the following to determine your eligibility on the sliding fee scale:

– Proof of income (paystub, unemployment check, letter from social security, tax return)

– Proof of address in LA County (utility bill, lease agreement)

– Photo identification

Please remember, you will not be denied services based on your income or citizenship status.
